Key Responsibilities- Deliver Patient-Centered Care: Provide personalized care to patients, focusing on their unique needs and preferences.
- Assess and Plan Care: Conduct thorough assessments of patients' physical and emotional needs, and develop and implement individualized care plans.
- Administer Medications and Treatments: Safely administer medications and treatments as prescribed by physicians, and monitor patients' responses to care.
- Monitor and Report Changes: Continuously monitor patients' conditions and report any changes or concerns to physicians and other healthcare professionals.
- Collaborate with Healthcare Team: Work closely with physicians, registered nurses, and other healthcare professionals to ensure seamless care coordination and communication.
- Document Patient Information: Accurately and thoroughly document patient information, including medical history, treatment plans, and progress notes.
- Maintain a Safe Environment: Ensure a safe and clean environment for patients, including proper use and maintenance of equipment and supplies.
- Education and Certification: Current LPN licensure and certification in the state of New Jersey.
- Experience: At least 1 year of experience as a Licensed Practical Nurse in a home care or similar setting.
- Skills: Excellent communication and interpersonal skills, ability to work independently and as part of a team, and strong attention to detail and organizational skills.
