Job Summary

We are looking for a caring and compassionate physical therapist who will work with us to develop and promote their own niche practice. As a member of our team, you will have the opportunity to work with a variety of patients and develop customized plans of care to achieve optimal outcomes.

Responsibilities
  • Review physician referrals and notes regarding patient's condition
  • Perform initial evaluation for treatment, making associated recommendations based on exam findings
  • Design and execute customized plans of care to achieve optimal outcomes
  • Support treatment with timely documentation utilizing EMR software
  • Provide patients with necessary education on conditions, expectations, limitations, HEP, etc.
  • Coordinate with other providers (PT and PTA) to deliver appropriate care
  • Establish discharge guidelines including follow up recommendations
Requirements
  • Doctor of Physical Therapy (or commensurate)
  • Post-doctorate clinical experience
  • Current, unrestricted Indiana State licensure
