Home Health Physical Therapist Opportunity in Baltimore

We are seeking an experienced Physical Therapist to join our team at BAYADA Home Health Care in Baltimore, MD. As a Physical Therapist with us, you will have the opportunity to work with adult and geriatric clients in their homes, providing personalized care and support.
The estimated salary for this role is $120,000 - $150,000 per year, depending on experience. About the RoleAs a Physical Therapist with BAYADA Home Health Care, you will be responsible for making home visits to clients in designated geographic territories, performing diagnostic tests and measurements, developing and implementing individualized care plans, and educating clients and family members on rehabilitative care and activities necessary to promote healthy living.
Responsibilities- Making home visits to clients in designated geographic territories.
- Performing diagnostic tests and measurements, such as mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments.
- Developing and implementing individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
- Continually assessing and revising the Physical Therapy care plan, and participating with nursing in the multidisciplinary care plan, as appropriate.
- Educating and instructing clients, family members, or other client representatives, in rehabilitative care and activities necessary to promote the client's health, safety, and independent living.
- A current MD Physical Therapist license.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by school transcript or diploma.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem-solving skills.
- Solid computer skills; prior experience with electronic medical records (EMR) preferred.
