Key Responsibilities- Conduct Comprehensive Resident Assessments: Conduct thorough assessments of each resident's medical, physical, and psychosocial needs to develop individualized care plans.
- Coordinate Interdisciplinary Care Planning: Work with the interdisciplinary team to develop, review, and revise care plans to ensure residents receive comprehensive and coordinated care.
- Lead Nursing Staff: Provide guidance, support, and oversight to nursing staff to ensure high-quality patient care and adherence to facility policies and procedures.
- Monitor Resident Status: Daily monitor resident status, including lab results, incident reports, and physician orders, to identify potential issues and implement interventions as needed.
- Participate in Discharge Planning: Collaborate with the interdisciplinary team to develop discharge plans that meet the resident's needs and ensure a smooth transition to post-acute care.
- Current RN or LPN License: Must have a current and valid RN or LPN license in the state of Kentucky.
