- Assess patient needs and develop individualized care plans to promote efficient throughput and prevent readmissions.
- Collaborate with healthcare teams to ensure medical necessity and compliance with regulatory requirements.
- Provide education and support to patients, families, and caregivers to promote health literacy and self-management.
- Stay up-to-date with industry developments and best practices in case management and care coordination.
- Graduate of an accredited School of Nursing.
- 2 years of acute hospital patient care experience, with a focus on utilization and case management.
- Possession of current Texas State license for Registered Nurse.
- Active healthcare provider Basic Life Support certification (ARC or AHA).
- Bachelors or Masters Degree in Nursing.
- Accredited Case Management (ACM) certification.
