- Conduct Comprehensive Patient Assessments: Conduct thorough medical history reviews, physical examinations, and diagnostic testing to identify patient needs and develop individualized treatment plans.
- Develop and Implement Treatment Plans: Collaborate with healthcare professionals to create and implement treatment plans that meet patient needs and medical guidelines.
- Monitor and Evaluate Patient Progress: Regularly assess patient progress, adjusting treatment plans as necessary to ensure optimal outcomes.
- Provide Patient Education and Support: Educate patients and their families on disease management, medication adherence, and lifestyle modifications to promote healthy outcomes.
- Collaborate with Healthcare Professionals: Work closely with physicians, nurses, and other healthcare professionals to ensure coordinated and efficient patient care.
- Document Patient Encounters: Accurately document patient encounters, including assessments, interventions, and outcomes, using electronic medical record systems.
Requirements:
- 6 Months of Long-Term Care Experience
- DEA License
- Nurse Practitioner License
- Experience in Gastroenterology
