Job Description

About the Role

As a Social Worker II, you will serve patients and families in our community and beyond by helping them during a challenging time in their lives. Our Social Workers enjoy the variety of cases they are presented with, as there is always something new and different.

Key Responsibilities
  • Collaborate with a large team of other Social Workers to gain knowledge and experience
  • Work with an interdisciplinary team made up of nurses, therapists, physicians, and dieticians
  • Plan care to effectively meet the patient/family transitional needs
  • Monitor the length of stay and promote community resources
What We Offer

We provide supervision for new grads in-house or will reimburse up to $175/month for external supervision. Our part-time benefits include medical, dental, PTO, retirement, employee discounts, and more Tuition reimbursement and college grant programs are also available.

Requirements
  • Master's Degree of Social Work required
  • Familiar with laws regarding confidentiality, vulnerable adult, and patient's rights
  • Licensed Graduate Social Worker (LGSW) In State of Minnesota
  • American Case Management Association ACMA Case Manager-SW Certification or obtain among hire within 5 years
  • 1 year or more of clinical experience as a social worker preferred
