Key Responsibilities
  • Patient Assessment: Conduct thorough assessments of patients to determine their suitability for home infusion therapy, including reviewing medical records, evaluating lab results, and assessing the patient's overall health status.
  • Collaboration with Healthcare Providers: Work with healthcare providers, including physicians, nurses, pharmacists, and discharge planners, to coordinate the transition of patients from hospital or clinic-based care to home infusion therapy.
  • Medication Management: Review prescription orders for accuracy, completeness, and appropriateness to ensure that the prescribed medication is safe and effective for the patient.
  • Inventory Management: Oversee inventory management, including ordering and maintaining an adequate supply of infusion medications and related supplies.
  • Education and Training: Educate patients and their caregivers on the proper administration, storage, and disposal of medications, as well as potential side effects, and the importance of adhering to the prescribed treatment care plan.
  • Emergency Response: Be prepared to respond to emergency situations, such as adverse drug reactions, line infections, and equipment malfunctions, to ensure patient safety.
Requirements
  • Experience as a Clinical Pharmacist with TPN experience.
  • Strong knowledge of pharmaceutical care and home infusion therapy.
  • Excellent communication and collaboration skills.
  • Ability to work independently and as part of a team.
  • Strong problem-solving and critical thinking skills.
