Job Summary:

We are seeking a highly skilled Pharmacy Liaison to join our team at Nuvance Health. As a Pharmacy Liaison, you will play a critical role in providing support to our Ambulatory Care Pharmacy and Value-Based performance teams. Your primary responsibility will be to manage electronic prescription pools, assist with medication refills, and provide exceptional customer service to patients and healthcare teams.

Key Responsibilities:

• Manage electronic prescription pools and assist with medication refills according to protocols.
• Provide customer service and communicate with providers and office staff in person and via the EMR.
• Document medication prior authorization assistance and work with patients and healthcare teams for medication assistance guidance.
• Review patient charts and records for pertinent information related to medication refills.
• Facilitate high-level customer/patient service by working with patients for medication assistance guidance, including communicating with patients about refill status, provider appointments, adherence, and PAP programs.
• Communicate and collaborate with providers and office staff in person and via the EMR.
• Schedule appointments, patient outreach for medication adherence support, and use payor and internal data to identify patients with medication-related needs.
• Process medication refills according to protocols and ensure compliance with all HIPAA regulations.
• Obtain prior authorizations from intermediaries and payers, including managed care companies, insurance companies, or others as appropriate.
• Maintain thorough documentation within the EMR for medication prior authorization assistance requests, prescription confirmations, and other data by facility.
• Conduct pharmacy benefit checks at the providers' requests and maintain compliance with all pertinent regulations and laws.
• Assist with general resource support as needed and provide cross-coverage to colleagues as needed.
• Attend and participate in appropriate medication practice and pharmacy resource-specific meetings.

Requirements:

• High school diploma/GED and Registered Pharmacy Technician certification.
• Pharmacy Technician Certification Board (PTCB) or the National Healthcareer Association (NHA) certification within 6 months of hire.
• 1 year of experience as a Pharmacy Tech or similar healthcare position preferred.
