Key Responsibilities:

  • Develop and implement individualized speech therapy plans of care, adjusting as needed to ensure optimal patient outcomes.
  • Utilize activities and home exercise programs to improve speech and communication functions, promoting independence and quality of life.
  • Counsel patients and families on disease/injury management, providing education and support to enhance overall well-being.
  • Complete all clinical documentation in accordance with agency protocol and Medicare/Federal guidelines, ensuring accurate and timely reporting.

Requirements:

  • Current licensure as a Speech Therapist in the state of employment.
  • Minimum one year of experience as a Speech Therapist in an acute care setting, with home care experience preferred.
  • Valid driver's license and auto insurance, with the ability to meet all physical demands of the role.
