Job Description

As a Nurse Care Specialist, you will be responsible for implementing physician-prescribed plans of care, assessing patients, and administering medications and treatments. You will also coordinate with family members and other healthcare professionals to ensure seamless care delivery.

You will work closely with our experienced case managers to develop and implement individualized care plans that meet each patient's unique needs.

Required Skills and Qualifications

  • Active, unencumbered license (RN/LPN/LVN) in the state of Texas
  • Current CPR certification from American Red Cross or American Heart Association
  • TB skin test (current within last 12 months)
  • Six months prior hands-on nursing experience preferred but not required
