Laconia Nursing Home MDS Coordinator Opportunity

We are seeking an experienced part-time RN MDS Coordinator to join our team at Laconia Nursing Home. The primary purpose of this position is to coordinate the RAI process, ensuring timely and complete completion of the MDS, CAA's, and Interdisciplinary Care Plan.

Responsibilities:
  • Monitor and guide the completion of assessments in a timely manner
  • Supervise the inputting of MDS assessments, transmission to the state, and obtain receipt of validation
  • Schedule and facilitate care plan conferences and monthly QM meetings
  • Manage the Care Planning Process
Qualifications:
  • Nursing Degree from an accredited college or university
  • Current, unencumbered, active RN/LPN license and CPR
  • At least 2 years' experience in Long Term Care as an MDS Coordinator
Requirements:
  • Outstanding knowledge of Case-Mix, Federal Medicare PPS process, and Medicaid reimbursement
  • Excellent written and verbal communication skills and critical thinking abilities
  • Solid understanding of current federal, state, and local standards, regulations, and guidelines that govern our facility
  • Ability to organize and prioritize
