Join Our Team as a Home Health Physical Therapist

We are seeking a skilled and compassionate Home Health Physical Therapist to join our team at CenterWell Home Health. As a Home Health Physical Therapist, you will play a vital role in helping patients recover from illness, injury, or surgery, and regain their independence in the comfort of their own homes.

Key Responsibilities:
  • Develop and implement personalized physical therapy treatment plans to help patients achieve their goals and improve their quality of life.
  • Conduct thorough assessments and evaluations to identify patients' strengths and weaknesses, and develop targeted interventions to address their needs.
  • Collaborate with patients, families, and healthcare teams to ensure seamless care coordination and communication.
  • Provide education and training to patients and their caregivers on proper techniques and strategies for managing their conditions and promoting healthy lifestyles.
  • Document patient progress and outcomes, and participate in quality improvement initiatives to ensure the highest level of care.
Requirements:
  • Master's degree in physical therapy or related field.
  • Current licensure to practice physical therapy in the state of employment.
  • At least one year of experience as a physical therapist in a home health or similar setting.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Strong analytical and problem-solving skills, with the ability to think critically and make sound judgments.
Preferred Qualifications:
  • Doctorate degree in physical therapy or related field.
  • Bilingual or multilingual skills.
