Job Summary

We are seeking a skilled and compassionate NOC Shift Charge Nurse to join our team at Los Altos Sub-acute and Rehabilitation Center. As a key member of our nursing staff, you will be responsible for overseeing and supporting our Certified Nursing Assistants (CNAs) to ensure the delivery of high-quality patient care.

Key Responsibilities
  • Lead and support CNAs in providing timely and effective patient care
  • Interpret and clarify medical instructions from physicians and the Director of Nursing
  • Chart and report on patient condition changes, medication administration, and treatment regimens
  • Collaborate with the DON and Administrator to identify and implement process improvements
  • Model a kind, collaborative, and supportive work environment
Requirements
  • Current LVN license valid in California
  • 1-2 years of experience in a nursing or similar facility
  • Excellent customer service and emotional maturity skills
  • Outstanding communication and organizational skills
