Responsibilities- Provide direct patient care to patients with acute and chronic kidney disease, including those requiring dialysis and other life-sustaining therapies.
- Collaborate with interdisciplinary teams to develop and implement individualized patient care plans.
- Conduct thorough assessments, identify patient needs, and develop effective care strategies.
- Administer medications, treatments, and other interventions as prescribed by physicians.
- Monitor patient progress, report changes, and adjust care plans as needed.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions.
- Current Registered Nurse (RN) license in the state of practice.
- Associates Degree in Nursing (A.D.N) from an accredited school of nursing or Bachelor of Science in Nursing (B.S.N).
- Ability to work in a fast-paced environment, prioritize tasks, and manage multiple responsibilities.
