About the Role

As a Licensed Practical Nurse at Consulate Health Care, you will be responsible for providing direct patient care under the supervision of a professional nurse, focusing on activities of daily living. You will also assist in implementing individualized treatment plans and supervise Certified Nursing Assistants (CNAs).

In addition to your clinical responsibilities, you will participate in Falls Program and Pressure Sore Program management, ensuring that our residents receive the highest level of care possible.

Key Responsibilities Include:

  • Providing nursing services and conducting thorough evaluations of residents' medical status.
  • Documenting resident information accurately and maintaining confidentiality.
  • Working collaboratively with the healthcare team to deliver exceptional patient care.

Requirements

  • Current, unencumbered LPN license and active state registration.
  • Experience in Skilled Nursing/Rehabilitation facilities preferred.
  • Compassionate and dedicated approach to patient care.

Estimated Salary: $65,000 - $85,000 per year
