At CenterWell Home Health, we are seeking a skilled Physical Therapist to join our team in providing high-quality care to patients in their homes. As a Physical Therapist, you will play a vital role in helping patients recover from injuries, illnesses, and surgeries, and regain their independence.

The ideal candidate will have a degree from an accredited Physical Therapy Program and a current and unrestricted Physical Therapy license. A minimum of one year of physical therapy experience is preferred, but not required. Strong organizational and communication skills are essential for this role.

As a Physical Therapist with CenterWell Home Health, you will have the opportunity to work with a diverse range of patients and develop a personalized care plan for each individual. You will work closely with our interdisciplinary team to ensure that patients receive comprehensive care and support.

We offer a competitive pay range of $84,600 - $116,300 per year, as well as a comprehensive benefits package, including paid time off, 401(k) retirement savings plan, and employee assistance program. Key Responsibilities:

  • Plan and administer prescribed skilled physical therapy treatment and training for patients.
  • Test/screen the patient's physical strengths to assist the physician in evaluating the patient's level of function.
  • Develop/implement a conditioning/rehabilitation program consistent with the physician's Plan of Treatment.
  • Confer with the physician and clinical team members to obtain additional patient information and assist in developing, implementing, and revising the therapy treatment program.
  • Provide Physical Therapy Assistants and Home Health Aide staff with written instructions/care plan that reflects the current plan of care.
  • Accurately, promptly, and thoroughly document patients' care observations, interventions, and evaluations.

Requirements:

  • Degree from an accredited Physical Therapy Program.
  • Current and unrestricted Physical Therapy license.
  • Minimum of one year physical therapy experience preferred.
  • Strong organizational and communication skills.
  • Valid driver's license, auto insurance, and reliable transportation.
