- Conduct assessments and develop individualized treatment plans for patients
- Implement and evaluate interventions to promote patient recovery and independence
- Collaborate with multidisciplinary teams to ensure seamless care coordination
- WA state licensure
- 1 year of skilled home health experience
- BLS certification
- Oasis certification preferred
