**Simple Steps LLC**: We are seeking a skilled Home and Community Support Professional to join our team. The primary focus of this role is to provide high-quality home and community services to our Veterans with neurological disorders, physical disabilities, and traumatic brain injuries.

As a Neurological Disorder Support Specialist, you will be responsible for delivering quality client care plans, ensuring resources are engaged effectively to meet client needs, orienting/educating clients and their families about health care plan services and procedures, and supporting accurate and complete documentation.

The ideal candidate will have experience working with physical and cognitive impairments/disabilities, Traumatic Brain Injury, and Post-Traumatic Stress Disorder. You will also have a valid driver's license, reliable transportation, and agency-required auto liability insurance.

Key Responsibilities:

  • Provide home and community services to Veterans with neurological disorders, physical disabilities, and traumatic brain injuries
  • Deliver quality client care plans and ensure resources are engaged effectively
  • Orient/educate clients and their families about health care plan services and procedures
  • Support accurate and complete documentation
  • Transport clients during client visits

Requirements:

  • Experience working with physical and cognitive impairments/disabilities
  • Experience working with Traumatic Brain Injury and Post-Traumatic Stress Disorder
  • Valid driver's license and reliable transportation
  • Agency-required auto liability insurance
