About This Position

We are seeking an experienced Clinical Care Manager to join our team in New York City. As a key member of our healthcare recruitment and staffing firm, PRIDE Health, you will play a vital role in delivering exceptional patient care and coordination services.

This 13-week contract position offers a unique opportunity to leverage your leadership skills and experience in care management to drive positive outcomes for our clients and their patients.

Key Responsibilities
  • Develop and implement effective care plans that prioritize patient-centered care and optimize resource utilization.
  • Collaborate with multidisciplinary teams to ensure seamless transitions of care and minimize hospital readmissions.
  • Analyze data and trends to identify opportunities for process improvement and cost reduction.

With a strong background in nursing and leadership, you will thrive in this dynamic environment where no two days are ever the same.

Requirements
  • Five years minimum of clinical experience as a Registered Nurse.
  • Leadership experience preferred.
  • Experience with Care Coordination, Utilization review, and discharge planning Computer literacy required.
  • RN Licensed as a Registered Nurse in the State of New York required.
  • Baccalaureate degree required.
  • PRI and Screen certification within three months of employment is required.
  • Master's Degree and Bilingual Spanish Certification in Case Management preferred.
