Job Summary

We are seeking a skilled Admissions/MDS Nurse to join our team at Carespring. As a key member of our healthcare team, you will be responsible for ensuring the highest level of care for our residents.

Key Responsibilities
  • Oversee the MDS process, adhering to the RAI manual and ensuring accuracy in documentation.
  • Collaborate with other MDS nurses to complete initial assessments for new admissions, ensuring a seamless transition for residents.
  • Manage patient admissions scheduling, MDS assessment, and care planning, ensuring timely and effective implementation.
  • Submit required reports weekly or as directed by the Director of Nursing, maintaining compliance with regulatory requirements.
  • Review validation reports to ensure error-free submissions, upholding the highest standards of quality.
  • Obtain and review Quality Metrics (QM's) to inform care planning and improve resident outcomes.
  • Work closely with the Interdisciplinary Team (IDT) to enhance resident care and outcomes.
  • Attend IDT meetings to discuss Medicare and Medicaid matters, ensuring a comprehensive understanding of MDS processes.
  • Develop and update comprehensive care plans, addressing the unique needs of each resident.
  • Ensure non-nursing sections of the MDS and Care Area Assessments (CAA's) accurately reflect resident needs, promoting person-centered care.
  • Monitor care plans to identify resident needs, making adjustments as necessary to ensure optimal care.
  • Review order recap reports and nurses' notes to ensure care plans are followed and determine the need for additional OBRA MDS assessments.
  • Monitor residents' changes in status, determining the need for changes per RAI guidelines.
  • Ensure accuracy of supportive documentation, adhering to MDS/RAI regulations.
  • Collaborate with facility consultants to implement recommended changes, driving continuous improvement.
  • Complete necessary paperwork, including AIMS, Fall Risk scale, Braden, Progress note, Care Plan, and nurse aide care plan (Kardex) documentation.
Requirements
  • Current LPN or RN license in the state of Ohio.
  • A flexible work schedule and the ability to work independently.
  • An understanding of the MDS process and a willingness to continually learn and grow.
  • Knowledge of PDPM and RUG.
