The Surgery Case Manager will play a critical role in ensuring seamless transitions for patients undergoing surgical procedures. This position will be dedicated to the surgery team, working closely with clinical staff and medical providers to coordinate discharge planning and interventions.
Key Responsibilities:- Conduct thorough assessments to identify patient needs and develop individualized discharge plans.
- Collaborate with the clinical team to initiate and implement discharge plans, ensuring timely and efficient transitions.
- Stay up-to-date on changing organizational, regulatory, and payer requirements to ensure compliance and optimal care outcomes.
- Develop and maintain strong relationships with patients, families, and healthcare providers to promote patient-centered care.
- BSN preferred; ADN required
- 3-5 years related experience, with at least 2 years in case management with surgery or ICU experience
- Maryland Registered Nurse License
- Intent to achieve MD licensure if out of state
- Case Management certification preferred
