Location: Wilmington, Delaware, recognized as the largest city in the state, serves as its economic and cultural nucleus. Positioned in the northern region of Delaware along the Delaware River, it is part of the Philadelphia metropolitan area.

Facility: This program is dedicated to the philosophy that seniors with chronic care requirements and their families benefit most when served within their community. It caters to individuals aged 55 and older, certified by the state as needing nursing home care, who can safely reside in the community at the time of enrollment and live within a designated service area.

Opportunity:
  • The physician delivers primary medical care to participants, which includes assessment, collaborative care plan development with the interdisciplinary team, provision of direct medical care, and evaluation of the care plan's effectiveness.
  • Engages in a collegial relationship with other healthcare professionals, making independent decisions regarding medical needs and health regimens.
  • Executes medical procedures as necessary.
  • Participates as a member of the interdisciplinary team (IDT) in initial, semi-annual, unscheduled, and annual assessments; attends morning updates and communicates changes in participants' baseline status to relevant staff daily.
  • Conducts initial and ongoing participant assessments, develops care plans, and implements them.
  • Integrates the primary care treatment plan into the overall care plan developed by the interdisciplinary team, interacting with team members to address the emergent and acute needs of participants. Evaluates and treats participants during acute illnesses.
  • Refers participants to medical specialists as needed. Manages all aspects of participant care in the nursing home: conducts regular visits to oversee care in collaboration with the Interdisciplinary Team; maintains communication with nursing home staff for continuous monitoring of participants on a case-by-case basis; admits nursing home participants to hospitals when necessary. Oversees and manages participants' use of medical specialists and inpatient care, communicating and following up with specialists regarding visit outcomes.
  • Completes documentation and utilizes appropriate diagnostic coding.
  • Under the direction of the Medical Director, reviews medical outcomes/statistical and utilization data, including the necessity of hospitalizations and use of specialists. When issues are identified, collaborates with the Medical Director to establish best practices and works with clinical staff and the interdisciplinary team to implement necessary procedural or operational changes to minimize unnecessary hospitalizations and enhance participant health.
