As a Registered Nurse, Per Diem, you will work under the supervision of the Director of Patient Services and Nursing Supervisor, providing complete care to assigned clients within the scope of the State Registered Nurse Practice Act and the policies and procedures of the organization.

Key Responsibilities:
  • Coordinate resources and services for assigned caseload based on client needs and health conditions.
  • Complete comprehensive client assessments to implement the best plan of care for RiseBoro Homecare clients.
  • Implement the Plan of Care by ongoing assessment, planning, coordination, counseling, teaching, and supervision.
  • Deliver ongoing assessment, teaching, and training to patients/caregivers that preserves and protects patient autonomy, dignity, rights, values, and benefits.
  • Make recommendations regarding the continuation, modification, or termination of home care services and document them in patient records accordingly.
Requirements:
  • NYS licensed and currently registered as a Registered Professional Nurse.
  • Basic Cardiac Life Support certificate.
  • Basic MS Office Suite computer skills; database entry, email, etc.
  • A minimum of two (2) years of experience in home health care preferred.
  • Bilingual English/Spanish speaker preferred.
