Job Summary

We are seeking a highly skilled Medical Social Worker to join our team in Trenton, NJ. As a Medical Social Worker, you will play a vital role in providing social work intervention to patients who require assistance with social or emotional problems that may impact their medical treatment and recovery.

Key Responsibilities
  • Assess patient psychosocial needs and develop interventions to meet established patient goals
  • Collaborate with other disciplines to plan and evaluate team goals for each patient
  • Facilitate patient referrals to community agencies when necessary
  • Provide financial counseling to achieve treatment goals
  • Assist patients and families in planning for present and future psychosocial needs
  • Observe and report patient progress and changes in patient status to physicians and multidisciplinary team members
Requirements
  • Baccalaureate degree in Social Work (BSW) or Master's degree from a school of social work accredited by the Council on Social Work Education
  • Minimum of one year of social work experience in a healthcare environment
  • Compliance with all relevant professional standards of practice
  • Participation and completion of Amergis' Competency program
  • Current CPR certification, TB questionnaire, PPD, or chest x-ray, and current Health certificate
  • Ability to effectively communicate with patients, families, and healthcare team members
