Job Title: Registered Nurse - Hospice Care

BAYADA Home Health Care is seeking an experienced Registered Nurse to join our team as a Hospice Care Provider. As a key member of our care team, you will be responsible for providing high-quality, patient-centered care to individuals in need of hospice services.

Responsibilities:
  • Make home visits to clients in designated geographic territories after business hours and during weekend days and evenings.
  • Perform assigned duties, including administration of medication, wound care, treatments, and procedures, and comfort care.
  • Monitor clients' conditions and report changes to Clinical or Client Services Manager.
  • Follow up with, execute, and properly document doctors' orders.
  • Perform client assessments as necessary.
Qualifications:
  • A current license as a Registered Nurse in VT and NH.
  • A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
  • Prior home care and/or hospice experience strongly preferred, but not required.
  • Solid computer skills; prior experience with electronic medical records (EMR) preferred.
  • Ability to travel to cases as assigned.
