Job Description

Eckerd Connects is a leading provider of behavioral health services, and we are seeking a highly motivated Youth Advocate to join our team. As a Youth Advocate, you will play a critical role in supporting our clients in achieving their goals and improving their overall well-being.

About the Role
  • This is a salaried position with an estimated hourly rate of $17.00 based on a 40-hour workweek.
  • The ideal candidate will have experience working with at-risk youth and families, with a strong background in social work or a related field.
  • You will be responsible for facilitating wraparound processes, developing individualized plans of care, and coordinating services with multiple providers.
Key Responsibilities
  1. Facilitate high-fidelity wraparound processes for youth and families, providing individualized, coordinated care to meet complex needs.
  2. Work with Independent Evaluators to ensure comprehensive assessments and incorporate results into care planning.
  3. In partnership with families, develop individualized plans of care adhering to National Wraparound Initiative standards.
