  • Develop and Implement Care Plans: Develop and implement individualized care plans that meet the unique needs of each patient.
  • Communicate with Healthcare Providers: Communicate with healthcare providers, patients, and families to ensure that patient needs are met and that care is coordinated.
  • Monitor and Evaluate Care: Monitor and evaluate the delivery of patient care to ensure that it meets the highest standards of quality and safety.
  • Participate in Quality Improvement: Participate in quality improvement initiatives to ensure that patient care is continuously improved and that best practices are implemented.
Requirements
  • Current RN Licensure: Current RN licensure in the state of practice.
  • CPR Certification: Current CPR certification.
  • Driver's License and Vehicle Insurance: Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
