Job Summary

We are seeking a highly skilled Registered Nurse - Case Manager to join our team at Matrix Providers. As a key member of our healthcare team, you will be responsible for providing exceptional patient care and case management services to our Military Service Members and their families.

Responsibilities
  • Participate in peer review and performance improvement activities to ensure high-quality patient care.
  • Function with an awareness and application of safety procedures to maintain a safe and healthy work environment.
  • Perform efficiently in emergency patient situations, following established protocols and remaining calm under pressure.
  • Participate in the implementation of the Family Advocacy Program, including medical examinations, documentation, and reporting.
  • Exercise awareness and sensitivity to patient/significant others' rights, as identified within the MTF.
  • Participate in continuing education to meet own professional growth and stay up-to-date with the latest healthcare trends and best practices.
  • Attend and/or comply with all annual training classes required by the Command, including disaster training, infection control, Sexual Harassment, Bloodborne Pathogens, Fire and Safety, Chemical, Biological, Radiological, Nuclear and Explosives (CBRNE), and all other required training.
  • Actively participate in the command's Performance Improvement Program, identifying opportunities to improve care delivery and recommending corrective action when problems exist.
  • Provide in-service training to clinic staff members and support government employees, such as hospital corpsmen and students.
  • Perform timely, accurate, and concise documentation of patient care, operating and manipulating automated systems, and participating in clinical staff Performance Improvement (PI) and Risk Management (RM) functions.
  • Exercise appropriate clinical delegation of tasks and duties, directing and coordinating health care team members, patient care, and clinical activities.
  • Maintain documentation of all treatment provided, preparing records and reports as required, and ensuring all records and reports are legible and follow local instructions.
Requirements
  • Bachelor's Degree in Nursing from an accredited college or university.
  • Current, full, active, and unrestricted license to practice as a Registered Nurse.
  • One year of clinical experience as a full-time RNCM within the last 3 years working in a medical facility.
  • Possess one of the following certifications: CCM, CDMS, CRRN, COHN, COHN-S, ACCC, CRC, RN-NCM, or CMC.
