We are seeking a skilled Physician in Geriatric Care to join our team at Fallon Community Health Plan. As a Geriatric Care Physician, you will be responsible for providing medical oversight to patients in our facilities, coordinating with our interdisciplinary team, and participating in daily morning IDTs.
Responsibilities- Provide medical oversight to patients in our facilities
- Coordinate with our interdisciplinary team to ensure comprehensive care
- Participate in daily morning IDTs to discuss patient care and treatment plans
- Work collaboratively with our team to develop and implement individualized care plans
- Stay up-to-date with the latest medical research and best practices in geriatric care
- Medical degree with a focus on geriatrics
- Board certification in Internal Medicine, Geriatrics, or Family Practice
- At least two years of experience working with frail or elderly populations
- Previous management experience is desirable
