Position Overview: Tact Staff is currently seeking a skilled travel nurse RN specializing in Case Management within Acute Care settings.

- Duration: 13 weeks
- Weekly Hours: 36 hours
- Shift: 12-hour shifts
- Employment Type: Travel
Job Duties: The RN Case Manager will oversee Care Coordination, manage Care Transitions, and facilitate Discharge Planning throughout the acute care patient journey. Responsibilities include:
- Conducting timely Initial Assessments, maintaining updated notes, and finalizing documentation.
- Completing referrals to post-acute care facilities.
- Collaborating with a multidisciplinary team to ensure smooth discharge processes.
Qualifications:
- Valid RN Licensure in California
- BLS Certification (AHA)
- Proficiency in EPIC Systems
- Minimum of 1 year experience with knowledge of post-acute care options including Home Health, Hospice, Palliative Care, Skilled Nursing Facilities, Long-Term Acute Care, Behavioral & Community Health, Sub-Acute, and Acute Rehabilitation.
Shift Details: 4 x 8 Days from Friday to Monday, with no on-call duties required.
