Hospice Care Specialist Job Description

We are seeking a skilled Hospice Care Specialist to join our team at Patriot Hospice. This rewarding role offers the opportunity to work with patients and families, providing compassionate end-of-life care and support.

Responsibilities:

  • Evaluate patient eligibility for Hospice services and conduct face-to-face encounters.
  • Develop and implement individualized care plans in collaboration with the Interdisciplinary Team.
  • Provide medical care within your scope of practice to patients in their place of residence.
  • Collaborate with Hospice nurses to address patient care issues and consult with the Medical Director on complex cases.

Requirements:

  • South Carolina certification/License is required.
  • Prior experience in hospice care or long-term care is preferred.
  • Strong computer skills are necessary.

Salary Range:

$90,000 - $125,000 per year, depending on experience.
