Admission Nurse RN

We are seeking a skilled Admission Nurse RN to join our dynamic and innovative team at HospiceCare in The Berkshires. As a key member of our admissions team, you will play a critical role in ensuring a smooth and efficient patient experience.
Key Responsibilities- Maintain resident census consistent with the admission strategy, working closely with the Admission Coordinator.
- Develop strong relationships with IHCS Liaison, area discharge planners, and social agencies to promote our facility's reputation and encourage referrals.
- Facilitate and direct the admission process, ensuring timely and accurate completion of nursing assessments, care plans, and documentation.
- Provide exceptional patient care, including medication reconciliation, ordering medications from the pharmacy, and initiating Medicare forms on admission.
- Identify potential room-review for infections, equipment needs, and clinical issues, and notify Admissions of decisions.
- Greet residents to their assigned rooms, conduct assessments, and write nursing notes.
- Develop care plans based on assessments and physician orders, and create C.N.A. Care Cards.
- Report on resident status to Nurses and C.N.A.s, and complete/facilitate discharge planning and discharge processes.
- Write referrals to transferring agencies, ensure patients have a 7-day supply of medications, and review discharge paperwork.
- Must possess a Massachusetts RN License.
- Minimum of 2 years clinical nursing experience.
- Ability to read, write, speak, and understand the English language.
- Capacity to make independent decisions when circumstances warrant such action.
