We are seeking a skilled professional to fill the role of MDS Coordinator at Five Points at Lake Highlands Nursing and Rehabilitation. This position requires strong organizational skills, excellent communication abilities, and a solid understanding of nursing home regulations.

Job Description:

The successful candidate will be responsible for scheduling residents for assessments, ensuring compliance with federal and state regulations, and maintaining proficiency in software. Additionally, they will coordinate Medicaid meetings, ITTM meetings, and care plan updates as needed.

Responsibilities:
  • Schedule residents for timely assessments, including admission, annual, quarterly, Medicare assessment or change-of-condition assessments, and oversee communication to nursing and other departments.
  • Ensure completion and coordination of comprehensive assessments (MDS) in accordance with federal and state regulations.
  • Monitor and maintain accurate, chronological, and current documentation for all residents.
  • Maintain proficiency in software to efficiently manage tasks and responsibilities.
  • Cordinate regular Medicaid meetings to discuss current residents, pending residents, and recipients.
  • Cordinate ITTM meetings to gather information, communicate changes, and maintain clinical records.
  • Perform care plan updates as necessary.
  • Provide Quality Indicators Report to DON, Administrator, and pertinent staff.
  • Transmit MDS on a timely basis.
Requirements:
  • Licensed Nurse in good standing.
  • Geriatric nursing background.
  • Restorative care philosophy, excellent professional skill (assessment and hands-on), and good communication and organization skills.
  • Working knowledge of nursing home regulations.
  • Ability to work independently and collaboratively as a team member.
  • Excellent organizational skills to manage multiple projects simultaneously.
Salary:

$62,000 - $72,000 per annum
