**Job Summary:**
We are seeking a highly skilled Travel Nurse RN - Home Infusion Specialist to join our team at Mount Sinai Health System - 1111 Amsterdam Ave. As a Travel Nurse RN - Home Infusion Specialist, you will be responsible for developing and maintaining core therapy and service line admissions to home infusion through positive referral source relationships and responsive, caring customer service.
Key Responsibilities:
- Develops and maintains core therapy and service line admissions to home infusion: Develops and maintains relationships with referral sources to ensure a steady flow of patients to our home infusion services.
- Collaborates with referral sources and Mount Sinai Home Infusion staff: Works closely with referral sources and Mount Sinai Home Infusion staff to facilitate the coordination of care for new patient referrals to Mount Sinai Home Infusion.
- Coordinates and communicates all critical workflow activities: Coordinates and communicates all critical workflow activities for new patient referrals to Mount Sinai Home Infusion personnel and referral sources.
- Ensures all documentation is accurate and complete: Ensures all documentation is legible, accurate, and complete and complies with company policies and procedures, as well as regulatory and accreditation standards.
- Maintains up-to-date referral source records: Maintains up-to-date referral source records, as well as any other requested records in accordance with Mount Sinai Home Infusion's policies, procedures, and business goals.
- Assists with activities to achieve favorable net revenue and gross profit goals: Assists with activities to achieve favorable net revenue and gross profit goals for Mount Sinai Home Infusion.
- Represents Mount Sinai Home Infusion pharmacy in a professional and positive manner: Represents Mount Sinai Home Infusion pharmacy in a professional and positive manner.
- Attends and participates in office meetings, outreach meetings, and training programs: Attends and participates in office meetings, outreach meetings, and training programs.
- Coordinates and cross-trains departmental work duties: Coordinates and cross-trains departmental work duties to achieve maximum productivity and efficiency.
- Remains current with developments in the field of infusion therapy: Remains current with developments in the field of infusion therapy.
Requirements:

- Integrates relevant assessment and intervention skills: Integrates relevant assessment and intervention skills in the delivery of nursing care.
- Collaborates with the interdisciplinary healthcare team: Collaborates with the interdisciplinary healthcare team in the development, implementation, and evaluation of the plan of care.
- Prioritizes all aspects of patient care: Prioritizes all aspects of patient care including teaching, rounding, coaching, and planning 'after hospital care' and delegating to others as appropriate.
- Educates patients and caregivers: Educates patients and caregivers while anticipating needs and readiness to learn, about their plan of care, transition of care, promotion of health, and prevention of disease.
- Communicates effectively and professionally: Communicates effectively and professionally with patients, family, and all members of the Interdisciplinary Patient Care Team.

- Manual dexterity: Manual dexterity to perform a variety of clinical procedures relevant to practice area (i.e. venipuncture, urinary catheterization, cardiopulmonary resuscitation).
