Job Description

Job Summary

We are looking for a highly motivated and experienced Registered Nurse to fill a contract Case Management position. The ideal candidate will have a strong background in acute care case management and leadership experience.

Responsibilities
  • Provide case management services to patients in an inpatient hospital setting.
  • Develop and implement individualized care plans to ensure optimal patient outcomes.
  • Collaborate with healthcare teams to coordinate patient care and services.
  • Conduct thorough assessments and evaluations to identify patient needs and develop effective care plans.
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care transitions.
  • Manage and coordinate patient discharge planning and utilization management services.
  • Ensure compliance with regulatory requirements and industry standards.
Requirements
  • Current acute care case management leadership experience (2 years minimum required).
  • Inpatient hospital case management experience.
  • Discharge planning and utilization management experience.
  • Case management experience.
  • Full License to practice as a Registered Nurse in the State of Michigan or Full License (LMSW) to practice as a Social Worker in the State of Michigan.
  • Certification in Case Management (CCM) by the Commission for Case Management.
  • Bachelor's degree in nursing, healthcare management or related health care field required or a Masters of Social Work required.
