Responsibilities- Provide comprehensive geriatric care to patients in various long-term care, TCU, assisted living facilities, and complex care/home-based medicine settings.
- Collaborate with a supportive team to deliver patient-centered care and achieve optimal health outcomes.
- Participate in ongoing education and professional development to stay current with best practices in geriatric care.
- Contribute to a culture of excellence, compassion, and partnership within the HealthPartners organization.
- Master's degree in nursing or DNP from an accredited institution.
- Geriatric, adult, or family certification.
- Active MN nursing license.
- Geriatric experience preferred.
