- Collect and analyze data from patients and their families to inform care decisions.
2. **Care Planning:**
- Develop individualized care plans that meet the specific needs of patients, considering their age and health status.
- Adjust care plans based on ongoing evaluations and patient responses.
3. **Documentation:**
- Maintain accurate and timely documentation of patient care activities and outcomes.
- Ensure effective communication with the healthcare team and families regarding patient status and care plans.
4. **Patient Education:**
- Identify educational needs of patients and their families, providing resources and support as necessary.
- Advocate for patients by recognizing and addressing barriers to effective communication and care.
5. **Quality Improvement:**
- Participate in quality improvement initiatives and evidence-based practice to enhance patient outcomes.

Qualifications:
Education:
- Required: Associate's Degree in Nursing (ADN)
- Preferred: Bachelor of Science in Nursing (BSN)
Experience:
- Required: Acceptance into a new graduate program or completion of an acute care new graduate program.
- Preferred: One year of experience in acute care nursing.
Licensure/Certification:
- Required: RN license in North Carolina or a Compact state; BLS certification.
- Preferred: ACLS, PALS, NRP, CCRN certifications may be required within a specific timeframe of hire.
Work Environment:
- This position requires flexibility in scheduling and the ability to work onsite as needed. The role demands a commitment to fostering a supportive and professional work atmosphere while delivering high-quality patient care.
