Position Highlights:

  • Lead the nursing center's interdisciplinary Resident Assessment Instrument (RAI) process, ensuring compliance with guidelines for MDS completion, computer encoding, and electronic state transmission.
  • Contribute to the development of personalized resident Care Plans, assessing resident needs and anticipated outcomes in accordance with federal and state regulations.
  • Provide training for new interdisciplinary team members on clinical content and the electronic completion of relevant MDS items.

Qualifications:

  • Current LPN license in good standing.
  • Previous experience in a hospital, long-term care facility, or related healthcare environment is essential.
  • Knowledge of general, rehabilitative, and restorative nursing practices, along with relevant laws and regulations governing long-term care.
  • Proficiency in computer applications, with AccuMed experience preferred.
  • Prior experience as an MDS Coordinator is advantageous.
  • Experience in completing MDS assessments and Care Plans is required.
  • Demonstrated proficiency in RAI/PPS processes is necessary.
  • Current BLS/CPR certification is required.

Position Type: Full Time

Pay Target: $41.00 / hour
