Position Highlights:
- Lead the nursing center's interdisciplinary Resident Assessment Instrument (RAI) process, ensuring compliance with guidelines for MDS completion, computer encoding, and electronic state transmission.
- Contribute to the development of personalized resident Care Plans, assessing resident needs and anticipated outcomes in accordance with federal and state regulations.
- Provide training for new interdisciplinary team members on clinical content and the electronic completion of relevant MDS items.

Qualifications:
- Current LPN license in good standing.
- Previous experience in a hospital, long-term care facility, or related healthcare environment is essential.
- Knowledge of general, rehabilitative, and restorative nursing practices, along with relevant laws and regulations governing long-term care.
- Proficiency in computer applications, with AccuMed experience preferred.
- Prior experience as an MDS Coordinator is advantageous.
- Experience in completing MDS assessments and Care Plans is required.
- Demonstrated proficiency in RAI/PPS processes is necessary.
- Current BLS/CPR certification is required.
Position Type: Full Time
Pay Target: $41.00 / hour
