Case Manager

Pyramid Healthcare Inc is seeking a dedicated and compassionate Case Manager to join our team of behavioral healthcare professionals. As a Case Manager, you will play a vital role in guiding clients through their recovery journey, providing support and coordination to ensure successful outcomes.

• Coordinate client engagement through appointments, aftercare plans, schedules, and meetings in collaboration with the treatment team.
• Communicate and coordinate with referral sources and outside providers to ensure seamless care.
• Provide case coordination services for clients through individual sessions, group support, and coordinating transportation needs.
• Assist in developing aftercare plans, providing referral and aftercare provider contact, and addressing outside provider needs.
• Document client, family, referral, and outside provider contacts in Carelogic in accordance with program timelines.
• Provide onsite support for Enhanced Outpatient Sessions (EOS) appointments between clients and Client Navigation team.

• High School diploma or equivalent required.
• 1 year in a drug and alcohol or mental health setting preferred.
