Job Title: RN Per Diem Home Care

We are seeking a skilled and compassionate Registered Nurse to join our team at Bedrock Healthcare at Home. As a Per Diem Home Care RN, you will provide high-quality nursing care to patients in their homes, ensuring their comfort, safety, and well-being.

Job Summary:

The successful candidate will be responsible for assessing patient needs, developing and implementing individualized care plans, and coordinating with healthcare teams to ensure seamless care delivery. Key Responsibilities:
  • Assess patient needs and develop individualized care plans
  • Provide skilled nursing care to patients in their homes
  • Coordinate with healthcare teams to ensure seamless care delivery
  • Monitor patient progress and adjust care plans as needed
  • Communicate effectively with patients, families, and healthcare teams
Requirements:
  • Current New Jersey licensure as a Registered Nurse
  • Graduate of an accredited school of nursing
  • Valid state driver's license and reliable automobile
  • Proof of current automobile insurance
Working Conditions:

This is a per diem position, and the successful candidate will be required to work variable shifts, including evenings, weekends, and holidays. The RN will be responsible for maintaining a safe working environment and adhering to all agency policies and procedures.
