About the Role

As a Home Health Nurse Specialist, you will be responsible for implementing prescribed care plans, conducting patient assessments, and coordinating with medical teams to ensure seamless care delivery.

Responsibilities
  • Patient Assessments and Care Planning: Conduct thorough assessments to identify patients' needs and develop personalized care plans.
  • Medication Administration and Treatments: Administer prescribed medications and treatments as ordered by physicians.
  • Documentation and Communication: Maintain accurate records of patient care and communicate effectively with medical teams and families.
  • Health Education and Promotion: Educate patients and families on healthy habits and disease management techniques.
Requirements
  • Licensure and Certification: Hold an active, unencumbered license (RN/LPN/LVN) in the state where you will practice and maintain current CPR certification.
  • Experience and Skills: Six months of prior hands-on nursing experience preferred, but not required. Must have reliable transportation and a strong commitment to patient-centered care.
