Job Summary

The Physical Therapist will assess, plan, and implement rehabilitative programs to improve mobility, relieve pain, and increase strength for patients suffering from disease or injury. This position will utilize physical and functional assessments to identify level of dysfunction, determine a plan of care, and document patient progress.

Responsibilities

• Conduct evaluations of assigned patients in accordance with established standards of care and practice.
• Plan and provide treatment to assigned patients as appropriate in accordance with established standards of care and practice.
• Coordinate patient care with other clinical team members to ensure optimal patient care and communication.
• Educate patients and caregivers in treatment and care to promote active participation in continuing activities at home.
• Complete all required paperwork following Physical Therapy department policies and procedures to ensure accurate documentation of care provided and outcomes achieved.
• Monitor and evaluate outcomes related to assessment and plan of care.
• Maintain and enhance personal knowledge, as well as promote professional standards in Physical Therapy.

Qualifications

• Physical Therapist licensed in the state of Florida.
• BLS for Healthcare Providers (BLS) through the American Heart Association.
