About the Role

We are seeking a skilled Occupational Therapist to join our Home Health team at FC Compassus LLC. As an Occupational Therapist, you will work one-on-one with patients in their homes to empower them to live safely and productively.

Key Responsibilities
  • Assess patients' needs and develop personalized plans to improve their daily activities and quality of life
  • Collaborate with patients, families, and healthcare teams to ensure seamless care
  • Provide education and training on adaptive techniques and equipment to enhance patients' independence
  • Conduct home assessments to identify and address environmental barriers
  • Document patient progress and outcomes to inform care decisions
Requirements
  • Bachelor of Science in Occupational Therapy
  • Current State License as an Occupational Therapist
  • Valid Driver's License
  • Current CPR, negative TB screen, and Hepatitis consent/declination
  • Two years' experience as an Occupational Therapist in an acute care or rehabilitation setting
