Job Summary: We are seeking a highly skilled and dedicated Physical Therapist to join our team at HealthPRO Heritage.

Key Responsibilities:

• Conduct thorough assessments of residents to identify their physical therapy needs and develop individualized treatment plans.
• Provide high-quality physical therapy services to residents, including exercises, stretches, and other interventions to improve mobility and function.
• Collaborate with other healthcare professionals, including nurses, doctors, and occupational therapists, to ensure comprehensive care for residents.
• Document resident progress and treatment plans in a clear and concise manner.
• Participate in interdisciplinary team meetings to discuss resident care and develop strategies for improving outcomes.
• Stay up-to-date with the latest physical therapy techniques and best practices through ongoing education and training.
• Contribute to the development and implementation of physical therapy programs and services at HealthPRO Heritage.

Requirements:

• Master's degree in Physical Therapy from an accredited program.
• Current licensure in the state of practice.
• Experience working with older adults and individuals with physical disabilities.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Strong problem-solving and critical thinking skills.
