Clinical Pharmacist Ambulatory(Internal Medicine) Position

We are seeking a highly skilled Clinical Pharmacist to join our team in Ambulatory Care. As a Clinical Pharmacist, you will play a vital role in delivering optimal medication management across all dimensions of care, every time.
Responsibilities:- Partnership with NHMG providers and clinical staff to provide remarkable patient care.
- Medication therapy management and patient counseling.
- Promotes high quality, evidence-based, cost-effective pharmaceutical care.
- Precepting of residents (PGY1/PGY2), medical residents and students.
- Participation in pharmacy and NHMG committees and shared governance.
- Have a clear understanding of quality improvement principles.
- Contributes to process improvement projects.
- Supports the educational and professional development of the department and system.
- Ability to communicate effectively both orally and in writing.
- Demonstrating proficiency with equipment and software.
- 4 Year/Bachelor's Degree in Pharmacy, required. Doctorate of Pharmacy, preferred.
- Postgraduate year 1 residency or fellowship OR two years of experience as a registered pharmacist in health system pharmacy, required.
- Ambulatory/clinic experience strongly preferred.
- Current Pharmacy licensure in appropriate state(s) or must be completed within 30 days of start date by either examination or reciprocity, required.
- Board of Pharmacy Specialties Certification or other approved certification must be obtained and continuously maintained within 2 years of start date as applicable to the role; excludes PRN team members who may not be eligible to take certification exam, required.
