Longevity Health is seeking an Advanced Practice Provider to join our clinical services team. As an Advanced Practice Provider, you will be responsible for providing high-quality, patient-centered care to our members in long-term care facilities.
Key Responsibilities:
- Provide collaborative primary care and services to a panel of Longevity Health members.
- Develop and implement individualized plans of care for members.
- Facilitate health plan clinical operations functions, including prior authorization, Part A and B services, medication reviews, and quality gap closure.
- Serve as a trusted advisor to members, loved ones, and nursing home staff, and advocate for member needs.
- Deliver effective and frequent communication and coordination with members' PCPs, loved ones, and care team members.
Requirements:
- State unrestricted licensed Advanced Practice Nurse or Physician's Assistant.
- National Board certification as an Adult, Family, Geriatric, or Primary Care nurse practitioner or Physician Assistant.
- DEA License required.
- Active national provider number (NPI) and Medicare provider number.
- Valid state driver's license with a good driving record and proof of automobile insurance.
Education and Training:
- Master of Science in Nursing as a Nurse Practitioner from a nationally accredited program or Master's Degree from a nationally accredited program.
Knowledge and Experience:
- Minimum of 6 months experience as a Nurse Practitioner or Physician's Assistant, with some experience in geriatric or palliative care.
- Experience working in the nursing home environment is strongly desired.
- Computer/Technology proficiency required.
- Excellent clinical skills, assessment, diagnosis, treatment, prescribing, and communication skills.
