Job Description

Longevity Health is seeking an Advanced Practice Provider to join our clinical services team. As an Advanced Practice Provider, you will be responsible for providing high-quality, patient-centered care to our members in long-term care facilities.

Key Responsibilities:

  • Provide collaborative primary care and services to a panel of Longevity Health members.
  • Develop and implement individualized plans of care for members.
  • Facilitate health plan clinical operations functions, including prior authorization, Part A and B services, medication reviews, and quality gap closure.
  • Serve as a trusted advisor to members, loved ones, and nursing home staff, and advocate for member needs.
  • Deliver effective and frequent communication and coordination with members' PCPs, loved ones, and care team members.

Requirements:

  • State unrestricted licensed Advanced Practice Nurse or Physician's Assistant.
  • National Board certification as an Adult, Family, Geriatric, or Primary Care nurse practitioner or Physician Assistant.
  • DEA License required.
  • Active national provider number (NPI) and Medicare provider number.
  • Valid state driver's license with a good driving record and proof of automobile insurance.

Education and Training:

  • Master of Science in Nursing as a Nurse Practitioner from a nationally accredited program or Master's Degree from a nationally accredited program.

Knowledge and Experience:

  • Minimum of 6 months experience as a Nurse Practitioner or Physician's Assistant, with some experience in geriatric or palliative care.
  • Experience working in the nursing home environment is strongly desired.
  • Computer/Technology proficiency required.
  • Excellent clinical skills, assessment, diagnosis, treatment, prescribing, and communication skills.
