About the Role:
We are seeking a highly skilled Nurse Practitioner or Physician Assistant to join our Home-based Medical Care team in Windham, CT. As a member of this team, you will play a crucial role in delivering high-quality care to complex patients in the comfort of their own homes.
Duties and Responsibilities:
- Deliver primary care to patients with complex needs, including geriatric and palliative care
- Collaborate with interdisciplinary teams to develop and implement patient care plans
- Conduct regular assessments and evaluations to monitor patient progress and adjust care plans accordingly
- Prescribe medications and treatments as necessary to improve patient outcomes

Requirements:
- Master's degree in Nursing or a related field
- Current RN or NP/PA license in good standing
