Job Title: Pediatric Care Specialist

About the Job:

As a Pediatric Care Specialist at KurzSolutions, you will be working in a hospital setting, providing care to premature and ill newborns, children, and adolescents. This is a level 1 Trauma Center and a teaching hospital, with a total of 323 inpatient beds, 70 of which are dedicated to the Neonatal ICU.

Responsibilities:

- Participate in all phases of the Case Management Program (CMP) and ensure that the CMP meets established case management standards of care.
- Provide nursing expertise about the CM process, including assessment, planning, implementation, coordination, and monitoring.
- Develop and implement local strategies using inpatient, outpatient, onsite, and telephonic Case Management.
- Develop and implement tools to support case management, such as those used to identify patients and assess their needs, clinical practice guidelines, algorithms, CM software, and databases for community resources.
- Integrate CM and utilization management (UM) and integrate nursing case management with social work case management.
- Maintain liaison with appropriate community agencies and organizations.
- Accurately collect and document patient care data.
- Develop treatment plans including preventive, therapeutic, rehabilitative, psychosocial, and clinical interventions to ensure continuity of care toward the goal of optimal wellness.
- Establish mechanisms to ensure proper implementation of the patient treatment plan and follow-up post-discharge with patients.
- Provide appropriate health care instructions to patients and/or caregivers based on identified learning needs.

Requirements:

- Education: Must have a Master of Science in Nursing degree from an accredited college or university.
- License: Current, full, active, and unrestricted license to practice as a Registered Nurse in the District of Columbia, or willing to obtain a license in the District of Columbia within 24 hours of position offer.
- Experience: Must have seven (7) years clinical RN experience, and five (5) years Case Management Experience.
- Possess one of the following certifications: Certified Case Manager (CCM), Advanced Certification in Continuity of Care (ACCC), Certified Rehabilitation Counselor (CRC), or American Nurses Credentialing Center Nurse Case Manager (RN-NCM).
