Job Summary:

JobRialto is seeking a skilled Registered Nurse to join our team as a Case Manager. In this role, you will provide comprehensive care coordination and case management services to a diverse panel of patients with identified needs.

Responsibilities:

• Provide patient-centered care and coaching to patients to improve their health behavior and attain health-related goals.

• Work collaboratively with healthcare teams, including providers, nurses, social workers, and patients, to ensure seamless care coordination.

• Develop and implement individualized care plans to address patient needs and preferences.

• Coordinate with healthcare providers to ensure timely and effective care delivery.

• Monitor patient progress and adjust care plans as needed to ensure optimal outcomes.

Requirements:

• Graduate from an accredited school of nursing.

• Active, unrestricted licensure as a Registered Nurse in a state.

• Current state's driver's license with the ability to travel to hospitals, skilled nursing facilities, patient's homes, and other sites where patients receive care as needed.

• Full COVID-19 vaccination is an essential job function of this role. Candidates located in states that mandate COVID-19 booster doses must also comply with those state requirements.

Preferred Qualifications:

• Certification in Geriatric Nursing or Geriatric Case Management.

• 5+ years of nursing experience, preferably with older patients.

• Proficient in computer use, the Internet, and health information technology.

• Demonstrated excellent communication, interpersonal, and organizational skills, with a flexible and creative approach to problem-solving.

• Proven ability to work effectively both independently and as a member of an interdisciplinary team.
